FOR THE 2025-2026 SCHOOL YEAR:
- New students must have the Health Inventory & Immunization Record completed.
- Current students who have had changes to their physical/mental health should have an updated Health Inventory & Immunization Record completed.
- Medication, inhaler, and allergy forms must be submitted every school year, along with their corresponding medication(s).

HEALTH FORMS (click link to download):
All health forms may be submitted via E-mail or hard copy.
ILLNESS POLICY (THE BASICS):
- FEVER (100.4 F+) - Students may return to school when they are fever-free for 24 hours without the use of fever reducing medication.
- VOMITING/DIARRHEA - Students may return to school 24 hours after their last episode of vomiting/diarrhea.
- COUGH/CONGESTION/COLD SYMPTOMS - Students are OK to attend school if they can adequately self-manage their symptoms (e.g., can properly cover cough/use tissues/wear mask/wash hands). Students who cannot adequately self-manage their symptoms should remain home until symptoms improve.
- COVID-19/FLU/RSV - Students should remain home. Please contact the school nurse - return-to-school criteria varies by case.
